TPS63000: Overheating

We are switching from a TPS63030 to TPS63000 due to part availability.  The TPS63000 part seems to work for a while but it gets

very hot and will eventually fail.  When it fails there is a low resistance from the Lx pins and ground.  The part is powered from a USB 5V

and we adjust the output between 2V and 5V.  Most testing so far has been with no load or a low load.  We only need 100ma from the device.

Same schematic but we added a cap to the input to reduce noise.  The PCB footprint changed for the new part.  So far we blew 3 parts.
