TPS63000: Burned Out when touch by hand

Hello.

I use TPS63000 and have some questions.

Condition1. Current conspumtion is under 100mA.

Condition2. Input voltage is 5.0v and Output voltage is 3.3V .

Q1. There is high frequency sound, when I touch by hand on operation.

       After that, It keeps draw very high current and seems like it's burned out and never recover. Is it normal?

Q2. I don't use R-C filter for 'VINA'. Is it okay? I don't see any information about this on datasheet.

Q3. There is high frequency sound when start-up sometime.

    A1: touch by hand is a necessary test for you?  it  is hard to say what happened during your touching, but i thing you can focus on ESD damaged or loop is lossing control.

          could you help to capture waveforms during your touching?

    A2: RC is not necessary.

    A3: i think you can capature waveforms during start-up and check if there is a frequency audible
