TPS62153: Intermittent TPS62153 Failures

I am seeing the same issues on 2 of 5 of my PCBs and was wondering whether there was a solution at all?

Basically, I power the board with 48V, I use a TPS54260 to regulate that 48V into 12V and the TPS62153 to regulate that 12V down to 5V. However, on the failed boards, the 12V appears to be shorted to the 5V line.

As I mentioned this is only on 2 boards that I've seen so far.

Is there a specific solution? Or is it simply that the IC may have been damaged in production and needs replacing?

I'm happy to share schematics and layouts if necessary.
