TPS63051: PCB fails when flashing MCU

We are developing a PCB using the next configuration for power management

The pcb is powered by a 3.7 V 400mAh 1.48 Wh Li-IOn battery, and powers a 32 bit mcu. an ftdi and an analog instrumentation circuit. We have done electrical tests without the mcu flashed and it works correctly, with stable current and eery non mcu dependent circuitry also working correctly. After flashing the MCU it worked for an hour and then some short circuit occurs between the TPS63051 and GND. We had a previous pcb with the same problem but we believed the error occurred due to stack-up issues but the same problem occurred with this new stack up. Due to supply chain issues we have very few prototypes so we would like to know any test we should do to find the problem.
