The datasheet has a Power-On sequence (Figure 10), but no Power-OFF sequence.
Do you have a Power-OFF sequence diagram?
It is designed with the following settings.
VIN = + 5V
A VDD = + 10.5V
VGH = + 18V
VGL = -6V
VCOM = 3.38V
FB4 = unused
