TPS54201: Output is wrong

Hi,

My customer is using the TPS54201 to do the test, and they found the output is wrong as below show. No matter how the duty cycle of the input PWM changes, the output waveform is similar to the attached picture as below, causing the load led light to flicker. could you help to check it? Thanks! 

More waveform about the circuit  that customer have test. CH1. SW CH2. Inductor current CH3. Output voltage and CH4. FB voltage.

and the schematic as below. The LED spec is 12V/2.4W LED, and the PWM input amplitude is 1.5V.
