If I understood correctly, TPS60400 was not correctly working on your board.

Can you please provide the following info:

Schematic of your application
Input voltage, output voltage, output current
Have you noticed this problem during internal testing? In how many boards have you noticed it?
Have you tried to replace the IC and check functionality?
What is the difference between the two voltage measurements (0.594 and 1.467)?
