BQ4050: BQ4050 XCHG=1 and XDSG=1 (Dis/charging disabled) without any obvious reason

Stuck on this. I've configured the 4050 for our application setting cell count, FET enable, capacity etc and for reasons not obvious to me I can't charge or discharge. If i use the "CHG_FET_TOGGLE" or "DSG_FET_TOGGLE" i can charge/discharge normally at high currents (everything looks good). I've read through the protection section of the datasheet and from what i can see any fault would be reported in the Battery Management screenshot below:

I kept a record of what we changed in the dataflash:

The only thing i can think of is the Design capacity or voltage is per cell and not for all 4 cells? The documentation isn't clear on this.
