Can you share the program you are using? Also, did you measure the rails on that have faults on your board to verify that it is the voltage that the GUI is reporting?

The above given issue has been solved by changing the power supply which i was using, but now i am getting a new error OTF and warning OTW. Can you help me with this?

Great news on the first issue. I have a few follow up questions:

1. Do you know what the thermal limit and warning are set to?

2. Do you know how much loading is on the output when these flags are set?

I am not sure about the thermal limit set but the output voltage was 0V when the flag came. i guess the load was 800mA.

It would be of great  help if you could share any guide explaining the flags. I tried surfing but could not find any.

Unfortunately, this is an older device and we do not have other documentation available besides the datasheet: https://www.ti.com/lit/gpn/ucd9248 

There is a section in the datasheet that covers the thermal options and different items that can be read from the PMBus. Please double check that you have the current limit set high enough on the power supply. It may also be helpful to check a second board to see if the behavior is consistent across more than 1 unit.
