Unexpected vLI shut down.

The LMG1210 is configured to the Independent Input Mode (IIT).

The green vLO signal is expected to copy the magenta vLI signal. However, vLo is distorted in the last four cycle and then is pulled down to zero. Meanwhile, the high-side gate signal vHO-vHs works well.

What cause this issue?

    Is the device still okay after this issue or does the IC break? It is unusual for LO to have issues while HO continues to work. Can you share some more details about the system including the operating frequency and voltage levels? Or a similar oscilloscope capture but with the the scales visible would work also. 

    Also, could you measure the DHL and DLH pins? One possibility is that noise can couple into the pins from the high slew-rate signals and cause the device to falsely enter PWM mode. The only thing this doesn't explain, is why the LO outputs seem to have latched off rather than just distorting. A schematic would also be helpful if you are willing to share.
