BQ24133: BQ24133 charging voltage abnormal

The customer design with bq24133, they set the cell to VREF, but the SW output voltage only 8.5V, and the STAT is blinking.

the schematic as below:

In 9.3.9, except that the delay of 1.5s has not been measured, it meets the requirements

Which will impact the SW output?
