LMR16030: Output is 400mV decaying over time to around 340mV.

I've tried using this IC to design 50->12V and 50->5V regulators, but have the same unexpectedly low output from both of them. Rather than the expected voltage, around 400mV is being generated, which gradually decays to 340mV then remains constant. I've assembled several of these now and the result is very similar. Are there any obvious flaws in my circuit? I'm still fairly new to this design process, and tried to base my circuit off of the WEBENCH designer.
