R4 is a series connection resistor between the VSNS- "negative remote sense" pin and the ground at the output of the converter. It is there to separate the VSNS- net from the ground net to ensure remote sense of the ground at the output. While not strictly required by the circuit, it ensures the remote sense ground line is not accidentally shorted to the ground plane at some other point, compromising the accuracy of the remote sense.
