TPS65580: Inquiry on the cause of device failure

Hello,

My customer has a defect while using the TPS65580 as shwon below and inquires.

Diagnosis of defective device:

 - Input Pin 1, 2 shorted to GND (about 10 ohms)

 - Output Pin 4 shorted to GND (about 10 ohms)

 - Appearance of defective device and operation of peripheral components are all normal

1. Please review whether there is a problem with the customer's circuit.

2. What could be the cause of damage like the symptoms above?

3. What problems can occur if the connection between POWER PAD and GND is incomplete?

4. Are there functions to protect the output voltage when overvoltage/overcurrent occurs on the input voltage pin?

    Thanks for reaching out! May I know customer/End equipment?

    1. Could you please let us know the failure rate? Is this failure happen on all boards or some of the boards?

    2. Do you see any burn out/damage of the defective device? Could you please also measure Pin15, Pin19 to GND?

    3. What is the input voltage range? Is it 5-18V? Could you please probe Vin, SW1, IL1 waveforms? Any overshoot happen?

    4. If connection of power pad and GND is incomplete, it will not good for thermal dissipation.

    5. There is strict requirement of Vin voltage range as you can refer to datasheet. There is damage risk if input voltage range exceed the ABS.

    Below is the customer's responses.

    1. This is a very low level compared to the quantity produced. It occurred on 2 boards.

    2. There is nothing special about the exterior. Pin4 to GND: 14.4ohm, Pin15 to GND: 5.2Kohm, Pin19 to GND: 29.8Kohm

    3. DC 12V. Shall I capture the input waveform of a normally operating device?

    5. Does that mean that up to 20V, which is the ABS range in the Datasheet, is fine, but from 21V onwards there is a problem?

    Thanks for your feedback.

    (1) The recommended operating range for input voltage is from 4.5-18V and ABS is -0.3-20V. Stresses beyond ABS may cause permanent damage to the device. 

    (2) Would you please share Vin, SW1, IL1 waveforms for the damaged device, any overshoot happen?
