BQ25619: Charging IC issue

With a VBUS of 12V, battery is not charging.  STAT pin is a constant low but 12V source shows low current being supplied, approximately 60 mA, when it should be 200 to 300 mA.  Inspection of registers appears to be correct to me.  Register 8 indicates charger is fast charging.  Furthermore, not only is the battery not being charged, it is draining as indicated by a decreasing battery voltage.

When VBUS is disconnected, the STAT pin remains low indicating battery is charging.  VBUS is sitting at 4.5V, same as the PMID voltage.  Below are the register settings as returned upon querying the charger when VBUS is connected to 12V.  I have also included some voltage measurements on the indicated pins.

I was unable to complete any measurements of charger voltages with VBUS at 12V as I inadvertently managed to short PMID and the charger no longer recognizes any VBUS input.  I will be getting a new board and try for more measurements.BQ25619 REGISTER SETTINGS.TXT
