LM5170-Q1: LM5170-Q1 IC failure

We've experienced this LM5170-Q1 IC failure in Buck mode and we don't know why. It seems related to rail voltage supplied to this IC as it's partially damaged. Resistance between Vcc to GND is only a few hundred ohms after this IC damaged.

I wonder if anyone experiences the same.

    I believe some pin exceeded absolute maximum ratings and caused damage.

    Will the glitch on DIR pin can cause the IC damaged? Datasheet shown there is a 10us glitch filtered on this pin? I know that VCC is very sensitive and has to be <12V. Any other pin that I need to pay attention with? What happens if there is no external connection link between AGND and PGND?

    Glitch filter for DIR pin is added to prevent errant operation due to the noise on the DIR signal.

    Please make sure absolute maximum ratings is always satisfied.

    I did not see application where  AGND and PGND are not connected.
