UCC27712: failing when powered ON

Hi, 

we are using UCC27712 as the gate driver for a High voltage isolated half bridge DCDC converter.

the regulator IC used is UCC28251.

we are facing frequent issue of gate driver failure (LO goes off) and HS to ground is shorted.

the MOSFET used is IPL65R115CFD7. 

we are having a filter of 60.4ohms and 47pf at both HI and LI.

please suggest whether any protection is needed for this IC?

    Thank you for reaching out! Can you help share the rest of the gate drive loop circuitry (FETs, outputs, bus voltage)? This will help the team get a better understanding of what could be causing the issue.

    Can you also help clarify what you mean by LO goes off? Do you mean that the output stops, or begins prematurely? Does it become shorted through the device? Can you share the switching frequency you are using for this application? Lastly, if you can help provide waveforms showing the behavior you observed it can help us identify the root cause.
