TPS56C230: TPS56C230 Vin short with PGND

Hi 

Customer TPS56C230 will be burned, and the specific performance is that Vin is short-circuited to PGND.

I checked the customer's layout and found that AVDD and PGND are connected under the chip without using 0 ohm resistors. Does this cause some risk Here?

    It is recommended to have a separate AGND island for all the small signal components. And, then connect this island to the PGND at a single point. The PGND pin is for the PGND plane of the input and output caps. See datasheet layout guidelines figure 35 for details. 

    VIN short to GND suggests the HSFET is likely damaged. It would be critical to see the entire schematic, layout and also what kind of testing was performed when the damage occurred? what phase is the project in and how many failures have occurred? Does replacing the IC solve the issue and does the design work after for a long time?
