We use the TPS65132B5YFFR with +/- 5.5V output preprogrammed. We don't programm the device. Around once every 20th startup the ouput voltage is +/- 4V instead of the +/- 5.5V. If the part is resetet with the enable pin, the voltage changes to the desired Voltage. To take away the load didn't help. Used circuit below:

The interval time between every startup is around 8 sec. But I did those power cycles so fast to reproduce the issue. We regognized it at power cycles with around 5min off time. The waveform below shows the output of a Part supplied by the TPS65132B5YFFR, with 4V supply it can't reach the 2.5V. The output of the TPS65132B5YFFR I only measured with a multimeter, therefore no waveform to share, but it follows the graph below just with other amplitudes.
