    Hello TKS 

    The cathode voltage of the DS404 can be greater than the input voltage only during transients when the input voltage falls down quickly. During normal operation, it is not possible to make the cathode voltage greater than 42V input voltage for a long time like as the screen capture in the DOCX file. 

    Please check if any signal or power rail is around the converter circuit, whose voltage is greater than the 42V.
