TPS92001: Shorted Light Drivers

I was directed to this website by our distributor Arrow Electronics. We are having problems with an LED driver TPS92200D1DDCR. About 7% of production lots are edning up shorted from Vin to GND. I would like some help in resolving this problem. I am more than willing to share design file so that we may discuss.

Please double check the part number. Is it TPS92001 or TPS92200?

The PN is TPS92200D1. I probably selected the wrong number from the drop-down menu.
