Can you describe the failure in more detail?

If you are willing to provide a schematic, we can offer to review the design to make sure the device was not operated out of spec.
If you are sure the device was handled properly and you suspect a quality issue with the LM2577 device, please open a support request for this case, so the inquiry is tracked and the quality issue process flow is started.
