LV2862: LV2862 heating up and going to thermal shutdown

We have made a custom board with LV2862 with 33uH inductor and DLFS1100 etc. We have device heating up badly and goes to thermal shut down.
If we power the device with 24V (output is 12V) it works well but still heats to around 45 degrees (external infrared reading).
If we raise the input to 40V or 42V the device goes to thermal shut down!!
Is this device recommended for 12V 500mA application with 32 to 42V input (10S cells)

    On your schematic, remove C6. There should not be a capacitor placed across the bottom feedback resistor. Sometimes, a feedforward capacitor is placed across the top feedback resistor.

    The GND connection in your layout is very thin and small for our device. The trace from your Boot capacitor to the switch cuts through the GND, depriving the IC of a path for heat to escape. I recommend following the EVM layout as closely as possible: https://www.ti.com/lit/ug/snvu729/snvu729.pdf

    I've attached the EVM layout as an example to show how the device has multiple paths for heat to escape.
