TPS2321: IC damaged

Hi.

We are connecting our TPS2321 to the TPS40054 and load-shared controller. Initially, everything was good, but now we are implementing an interposer board in the load share controller as we could only find Through-hole ICs. The issue we are encountering is that after testing the new modules, sometimes the IN2 (pin 10) is damaged, causing it to short-circuit to DGND (Pin 3). However, ISENSE2 (pin 7), which is connected to Pin (10), appears not to be damaged. Do you have any insights into what might be causing PIN10 to become damaged? Is overvoltage on the pin the only way to damage the IC?
