LM5164-Q1: Buck SW pin field failure in running vehicle

Dear TI team,

There was 10000 Board are running in field with LM5164Buck in BMS application, Battery maximum voltage is like 67.2V which is buck I/P, There are two buck failure return on filed in running vehicle. 

1st One: LM5164 SW pin resistance was 2.6k with respect to I/P & there was no output output.

2nd One: LM5164 I/P pin getting short with ground same case in field, There was also transient protection circuit in the design the TVS was safe .
