LM61495-Q1: a case of malfunction where the output was 12V

Hello,

As shown in the figure below, the normal output of the power supply is 5.5V, but we encountered a case of malfunction where the output was 12V.

After measuring the pull-up and pull-down impedances of the feedback, the malfunction disappeared. Could you please help analyze from the chip side what abnormal condition could have led to the 12V output?
