We are also noticing a short circuit at the SW pin. After some time of powering on the IC, the internal MOSFET appears to show a short circuit. Could you assist in diagnosing this issue

Please find the attached layout details for your reference. the highlighted  is boost convertor area in the Layout

The layout is far from satisfying and this will cause high voltage spike on low side mosfet. That's why you'll see short behavior between SW to GND. Please follow below link and optimize your layout.

Initially, everything was working fine. However, after a few attempts, the IC's internal low side MOSFET got damaged when the input voltage changed or decreased. We are unsure whether this was due to the layout or the inductor we used (1239AS-H-1R0M=P2). Initially, the input voltage and the output voltage were the same, for example, Vin was 4V and Vout was also 4V. When we observed the SW pin with a scope, we noticed continuous 25kHz spikes with the Vin voltage, instead of 1 MHZ PWM signal. Why is this happening?

Most likely low side MOSFET got damaged because of the poor layout. You can start by optimize your layout first. About the 25KHz, please share the waveform of Vin SW and Vout.
