The bootstrap circuit provides a regulated DC voltage to VDD. The DC value of the bootstrap is the amplitude of the gate drive so yes, it is required.

You can apply VDD from an external DC lab power supply. I would recommend to place a diode in series with the connection from V+ to VDD.

Good practice that whenever you bias externally, I like to introduce the voltage source through a diode. If you think about a bootstrap winding bias applied to VDD, this is also a voltage bias applied through a diode.
