I experience exactly the same problem here. The device can run for hours without a problem. After cutting and reapplying power the device blows out sometimes. This happened to multiple machine produced boards in a controllerd enviroment.
The last one blown out had nothing connected to it's output. So there was no current flowing.
The PCBs were produced by EuroCircuits, they also delivered the parts.
