LM5060-Q1: Vgs overvolting causing a short from Gate to Source

Hi,

The issue I have found is sometimes with certain loads (capacitative we think) Vgs overvolts, causing a directional short from Gate to Source on the main fets (FDBL9401-F085). With repeated testing, this evolved into a bidirectional short, with even less resistance. How do you think this Vgs surge is occuring? 

A spice model would be very helpful to simulate if anyone can point me to one.
