TPS562201: TPS562201 is burning

I made a +5V source with the TPS562201 CI for a project, but the chip is burning in some PCBs but

I am using TPS562201 in my project to generate +5V but the IC is burning in most of the PCBs and in few PCBs the IC is not burning.
What a try:
- to replace all capacitors, resistors and inductors to check the components used for the assembler are orginal.
- turn on the +5V source without anything conected to have a low load.
- turn on the +5V source with a 0.5mA load.
- Change the bench power supply.

    Circuit design looks OK.

    Did you tested on EVM? 

  • No, I did the tests on my own PCB project.
    To avoid interference from other circuits in my tests, I cut off the output signal of this source.

    A little more detail:
    As I mentioned before, some PCBs with the circuit work, while others do not.
    Here are two waveforms from my oscilloscope. The first one shows the circuit working, and the second one shows the circuit not working.
    The blue signal represents the input (12V), and the yellow signal represents the output (5V).

    In the second waveform, you can see that when the TPS562201DDCR tries to generate the 5V output, it creates noise in the input signal. The TPS562201DDCR is unable to generate the output, so it shuts itself off and attempts to restart. At this moment, the TPS562201DDCR burns out.

    It is important to mention that the power source I am using to generate the 12V can supply up to 3A, so the input current is apparently not the problem.

    Do you mean it's tested with no load? Did you swap the IC between normal and problem boards? 

    On problem board, the 12V input drop to very low level which mean the sink current over 3A. can you check the connection from P/S output to VIN pin of TPS562201 if there was too big wire resistance.
