LM5066I: Some LM5066i fault problem

I use LM5066I in my design as shown below, I refer to LM5066I EVB to design .The -48V is reference GND, so the input is 48V.

In some our sample run ,the +48Vin of all sample can output normally, but LM5066I of some sample didn't work after some working time, the +48Vin didn't out ,but when I only change a new LM5066I, the +48Vin can output normally again.

Please tell me is there any problem in my design, and would you fault analysis for our NG sample?

Hi Avishek,

We already send the NG units to our agent, and the agent FAE point out we need to add a Schottky diode on output in our design , I will continue to observe the condition after we add Schottky diode.
