LM5012: LM5012DDAR IC getting failed above 80V input supply while testing 500mA load

I am designing a power supply with a 12V, 2.5A output requirement. The input voltage can go up to 90V, so I selected the LM5012DDAR buck regulator for this purpose.

According to the datasheet, the LM5012 supports input voltages up to 100V for normal operation. However, during testing, the IC consistently fails when the input exceeds 80V.

Could you help me identify the possible reasons for this failure and suggest any solutions?
