TPSM63606: O/P Voltage issue

Hi ,

I have designed  a DC-DC Reg circuit Using the TPSM63606 IC.

Input voltage is 15VDC and the O/P voltage is 12V@6A Rating.  When i am doing the testing i am getting the O/P voltage as same as the I/P voltage. What ever input i am feeding  same thing i am getting as a output.

I have attached circuit for the ref, pls check and advice to solve this issue.
