thanks for reaching out in E2E.
Does the observed output short circuit condition mean that there is an IC internal short between PH pin and GND?
Input over voltage is a possible reason for device damage. I would suggest to check the input voltage for over voltage conditions.

I'm aware that an excessively high input voltage can cause damage, but currently there is no overvoltage. Other ICs are using the same voltage and have not been damaged.

sorry for the late reply, I was out for vacation.
I don't see any obvious mistakes inn the schematic.
I'm afraid that the condition, which caused the damage needs to be debugged in the application.
Could it be possible that the over voltage condition is handling related?
Is there any commonality between the affected devices?
Which signal pins are shorted?
