UCC384 Shutdown SC/DT issue

My customer is having an issue with shutdown feature of the UCC384DP- ADJ. The device goes to shutdown and does not recover again. The TTL controlled shutdown circuit is attached with this port. The date codes of the part which are failing are also attached here. Please check and advise if there is a problem with the design (selection of FET) or other. Currently the customer is pulling the SC/DT pin for workaround.
