Could it be the USB interface?

Is the green LED on the USB-to-GPIO adapter lit?

Connect the USB Adapter directly to host system port, avoid using USB Hub/Switch.

Confirm that you are not using the UCD9080 application with the UCD9081, although the devices are quite similar the GUI applications are not compatible.

Check that the Auto-Refresh selection is set in the File dropdown and/or try the Discover Devices selection.

All you mentioned  already checked.

customer  also connected scope to the adapter output lines, and it seems like it does not make any connection with the board.

What WIN OS are you using?

Have you tried this setup on more than computer?

Dose it work only with WIN 7?

There are two versions on the website, one works with WINXP and the other with WIN7

WIN 7 also dose not work. got same msg.

Change the USB Adapter setting: Change the bus speed to 100 kHz, Packet Error Checking = Enable.

How to change the USBsetting and the bus speed?

Currently the only method that I'm aware of is through a tool that is loaded along with the complete Fusion Digital Power Designer SW.  I tried the low level SW tools on the USB adapter webpage but they couldn't locate the adapter on my WIN7 machine.

After loading the Fusion Digital Power Designer, go to the Windows Start Menu and navigate to the FDPD folder, then the Tools folder under this and select the USB Adapter Settings Tool.

The only way I could replicate the same error message referenced above,

was to disconnect the USB cable from the USB-to-GPIO adapter.

If the USB cable was connected to the USB adapter but the ribbon cable wasn't connected to the EVM, then the application opened but indicated that "UCD9081 Not Present" in the bottom left corner.

Try another USB cable and use a USB port directly on the host system, avoid using a USB Hub/Switch.
