TPS84259 no output voltage, possible layout issue

We implemented a TPS84259 on a pc board and are not getting Vout.  The Vin is +24V.  The expected Vout is -15V.  I suspect a layout problem.  We had trouble understanding and following the information in the datasheet.  Some specific questions:

Should the PH pins all be connected together?

Should the PH pins be connected to anything?

I cannot post the schematic or layout, they are proprietary information.

 

Any general information would be appreciated.  I would also appreciate contact with a TI person to discuss more details.  My contact information:

    To answer your questions in short: Yes, all PH pins were connected internally. Nope, the PH pins should not be connected to any external component. Moreover, any "DNC" pin should be left unconnected.
