First, I am moving this post to the AC/DC Power Supply Forum, as it will receive proper attention here. Next, although this is not an exact design for your requirements, it is close and can probably be modified to meet your 12V 500mA request. Please review PMP5412: http://www.ti.com/tool/PMP5412 and also this Analog Journal article: www.ti.com/lit/an/slyt391/slyt391.pdf . Also note that PMP5412 does not use a transformer, however, it does have two inductors.
I will do some additional research for a Ref Design that may meet your requirements.

Using the TIDesigns page: http://www.ti.com/general/docs/refdesignsearchresults.tsp , here is a Ref Design that meets your 12V 500mA requirements: 85VAC to 264VAC Input, 12V/500mA Output Bias supply Reference Design (PMP20122).
There are several other designs also available, you may want to look through these for other suggested design considerations.
