I've been using the TPS60402DBVR in several designs to obviously produce a negative rail. They have all worked flawlessly until now. I'm seeing a high failure rate with the input, ground and output all being shorted in some combination (either input to ground, output to ground, or in some cases all three shorted together).
A new product I've been working on takes +24V in and drops it to +5V. The +5V is then inverted with the TPS60402 to -5V. The +/-5V is then used to power a quad op-amp (OPA4197).
The regulator has a 10uF input cap and 22uF output cap. The TPS60402 has a 1uF input cap, 3.3uF flying cap, and 3.3uF output cap. The -5V rail goes through a 10-ohm resistor, then a 10nF cap before the op-amp.
I've lost almost 20 of these charge pumps in this way and I'm unable to figure out why.
