LM2841: LM2841 Input Voltage Range Issue

I have used this device(LM2841YQMK/NOPBTR-ND) in several products. Based on the datasheet, it should support the voltage range of 4.5 V to 42 V. But, in our tests is has been broken in the voltage range of 30 V to 38 V. We have tested several of them and all of the were broken in a voltage smaller than 40 V. We followed the designs presented on the pages 13 and 14 of the datasheet.  Any similar experience, or suggestion?

    You may take a look on the EVM user guide for this device. www.ti.com/.../lm2841ymk-adjev. Efficiency data for 36V input is illustrated in the user guide, so the issue could be related with your design.

    You may check the voltage rating of components used in your design as first step. Then you may check whether the voltage stress on both VIN pin and SW pin can exceed the abs. max spec. shown in datasheet.
