TPS2413: Chip fails after a few months operation

We are attempting to use the TPS2413 ICs for a UPS circuit.  The battery power source is a 12V battery (operates at ~13.6V), and our main power supply source which is 15V.  

The TPS2413 at (U4) which is regulating the Battery power source fails after a few months of operation.  

We have a 2200 capacitor between terminals A & BYP, and in the scenario where the chip fails, we are able to recover functionality temporarily by draining the capacitor (shorting A to BYP).  But once all power is shutdown to the system, and restarted, it again goes into a failed state (always open).  

If we replace the TPS2413 chip with a new IC, the system is fully functional again.  

We have a few hundred boards built to date which are exhibiting the same problem.  

I've attached our schematic.  

Can anyone help us understand what we might be doing wrong?

    Only the TPS2413 fails, nothing else?  FET is good?

    A couple of things.  Gate source current is 290uA nominal.  Your schematic shows a 100k resistor feeding the base of Q2 to an LED.  Depending on the LED, you will have in excess of 200uA load on the gate. C10 may also be a problem. The fast turn off lasts for about 12us and when complete, the residual voltage on this cap may cycle the FET on again, resulting in further fast shut offs.  Best to remove the gate loading from the design except for the natural FET Ciss and Crss.

    Check the C voltage when you do a reverse current test where the 2A gate pull down is in effect.  It may climb over the max rating of the TPS2413 and you will need a transient suppressor on the output to prevent.
