The FET with above PN burned on top of packaging during qualification. have you seen this issue before?

Quynh,
There are a number of reasons why this could happen. Since the MOSFETs are responsible for driving the load, it is not uncommon for them to burn up if the design is unstable.

It sounds like your application needs to be debugged to find the source of that instability.
