    Please see my comments below:

    Since TPS2373 uses advanced startup the VIN pin of the UCC2897A should be open circuited.
    C16 should be changed to 0.1uF ceramic. While a 22uF cap should be moved to PVDD/VCOUT of the UCC2897A
    For J4, TPH/TPL/BT are referenced to RTN and not secondary ground. Therefore it should not connect to VOUT with jumper. Rather it should connect to VCOUT. Also rather than LEDs, you can consider opto to reference it to the secondary through the isolation boundary.
