PTH04040W: Burnout of FET

Hi, team.

Several events have occurred in which a specific FET of PTH04040W burns out.
The burning FET is circled in the figure below.

I think that other FETs will also break if input overvoltage, but only certain FETs are broken.
Can you estimate the cause?

I think that PTH04040 W is 3-Phase PWM, but is it always operating at 3-Phase? Or will the number of Phases increase depending on the load?

Hi Kobayashi-san,

What are your initial conditions(VIN,VOUT,IOUT)? The module that you've shown above uses a multi-phase switch-mode topology and always operates in 3-Phase allowing three paths to share the total load current.

Hi, Jimmy.

Vin=5V, Vout=1.2V, Iout=10-20A

The broken FET is always the same.
Is there a similar case?

Include your actual schematic with input and output capacitance ?

What is the output polarized capacitance ESR and types used in this application?

Can you include more details on the input power bus < are there any high voltages transients on the input bus
