TPS51225C: 3.3V and 5V LDO output ok，but 3.3V，pwm output is not ok

hi team，

I use TPS51225C in my design，the Schematic diagram is below：

the AC_IN=19V，when power in，VREG3 and VREG5 output is ok，but the P3V3_S5 is fail，

the Waveform of P3V3_S5 is blow：

the Waveform of pin10 DRVH2 is blow：

the Waveform of pin11 DRVL2 is blow：

Hi Anson,

How about P5V_S3? Is that looking good?

Is the AC_IN steady?

Also, what is the load when the shutdown happens?
