TPS23734: 25W at 5V Customized Design transformer selection

Hi TI Team,

I want to design customized PoE+ circuit for 5V@5A (25W Design). For that, I have found Evaluation module(MFG No.: TPS23734EVM-094) which has LinkComm Flyback transformer. From internet, I unable to find technical details of required transformer or procurement support.

Could you please suggest exact transformer requirement which I can send to transformer manufacturers or compatible transformer part for TPS23734 design?
