TPS2121: Failed ICs

Hello,

We are using TPS2121 on two products to handle a Primary and Backup 12Vdc power inputs. We worked with TI support and over a few board revisions came up with what I believe is a solid design. We have built 25 of each product and so far have found 3 failures. Two failed (stopped passing current) after 10 or so power cycles. A third failed device smoked after a dozen or so power cycles. So far we have powered up 29 of the 50 boards. None of the failed devices were presented with an overvoltage or reverse voltage condition. All of the devices were purchased from DigiKey in 2020 and have device marking 2121 / 05I / AF7Y. 

We are in the process of replacing the failed devices with new parts we are ordering today.

What other steps should we take to proceed? Should someone review our schematics again?
