TL4242: TL4242 without low-side sense

I wanted to reopen this question about finding some way to use the TL4242 without the low-side sense resistor.  I have been doing some testing with this part and it works really well, especially thermally - I am using the eval board and it runs very cool even when dropping about 1.5W.  Datasheet numbers make it look like even a larger SOIC-8 package has worse thermal performance.  The recommended high-side linear driver, TPS92613, is a much larger footprint, and too large for my application.

So, I know that the TL4242 is meant to be used with a shunt sense resistor.  But I'm wondering if you could recommend some way of using a high-side sense and converting it to low side for the TL4242?  

Or, perhaps another recommended driver, with high-side sense, a similar footprint, 500mA output, and similar thermal parameters?
