LM76003-Q1: LM76003-Q1 burn issue

hi,

Now i have another board having all the three LM76003-Q1 b urn out as shown below:

         

The test procedure are as follow:

1. The input is from a 48Vdc source which is connection to control 2units of 48V motor. At any one time, only one motor will be on (the other motor is for fail-safe).

2. the 48Vdc input source is connected to these 3 LM76003-Q1 which indirectly provide the split power rail for 1.8V, 3.3V, 5V supplier rails. 

3. At power on, when both motor is OFF, everything is fine. However, when one of the motor is ON, the LM76003-Q1 will burnt-out as shown in the pictures.

Thus, do you have any advice and suggestion why all the three LM76003-Q1 burn immediately when one of the motor is on? 

You advice is very much appreciated.
